Did the design run?
- Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
- No API failures: heldharness.apiNo model call failed at the API.
- Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
- No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
- Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
- No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
- Builtin tools: heldharness.builtin-toolsA check recorded with the run.
- Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
- No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
- The task was decomposed: heldorchestrator-workers.decompositionThe orchestrator’s reply was read through its schema and held at least one subtask.
- The fan-out happened: heldorchestrator-workers.fan-outEvery subtask got a worker, every worker ran, and the recorded subagent count matches.
- The synthesis wrote the answer: heldorchestrator-workers.synthesisThe final answer came from the synthesis call, not from a worker’s report standing in for it.
- Each call was shown the right tools: heldorchestrator-workers.toolsThe orchestrator and the synthesis were shown no tools and every worker the task’s full toolset.

Weekend queue triage: summary
All 14 weekend tickets have been triaged. Two were closed as not being service requests, and two pairs of duplicates were merged. The remaining 10 each have a severity from the matrix, a deadline and an engineer. Times are UTC, as of Monday 2026-03-23 08:10.
Needs attention now
Two tickets came in at 08:10 today, after triage began. They are not part of the weekend backlog, so they were left alone.
- T-2015, "Payslips missing for the Finance department" (payroll, one department). The matrix makes this sev1 with a 60-minute response target, so it would be due about 09:10 if triaged now. It belongs to the business-apps rotation, where E-201 is on call. It is not company-wide, so it is not a major incident. Someone should triage this right away.
- T-2016, "VPN token app asks me to register again" (vpn, single user). The matrix makes this sev4. Whoever triages it should first rule out phishing, because a prompt to register again is a common phishing lure.
Closed as not service requests
- T-2010: marketing spam
- T-2014: automatic out-of-office reply
Merged duplicates (the earliest report survives)
- T-2012 was merged into T-2003 (payments builds failing).
- T-2005 was merged into T-2004 (CRM search errors).
- Neither pair had a deadline already promised to the requester, so each survivor got the normal sev2 deadline.
Triaged tickets
| Ticket | Severity | Due | Assigned to |
|---|---|---|---|
| T-2001 Berlin VPN drops (team) | sev3 | Mon 17:00 | E-303 (network backup, covering) |
| T-2002 Saved CRM filter missing (single user) | sev4 | Thu 03-26 17:00 | E-203 (business-apps backup, covering) |
| T-2003 Payments builds failing (department) | sev2 | Mon 12:10 | E-101 |
| T-2004 CRM search errors (department) | sev2 | Mon 12:10 | E-201 |
| T-2006 Wrong tax code on payslip (single user) | sev2 | Mon 12:10 | E-201 |
| T-2007 Typed VPN password into a fake page | sev2 | Mon 12:10 | E-601 (security on-call) |
| T-2008 VPN client update fails (single user) | sev4 | Thu 03-26 17:00 | E-302 |
| T-2009 Build logs page slow (single user) | sev4 | Thu 03-26 17:00 | E-102 |
| T-2011 Warehouse overtime not imported (team) | sev2 | Mon 12:10 | E-201 |
| T-2013 Lisbon office cannot reach VPN (department) | sev2 | Mon 12:10 | E-303 (network backup, covering) |
Why some tickets differ from the plain matrix
- T-2006 is sev2 rather than sev3. Knowledge-base article KB-55 raises single-user payroll tickets to sev2 from 20 to 31 March, during the payroll close.
- T-2007 is a phishing report, so it is treated as a security incident: sev2, owned by the security on-call rotation.
Leave cover:
- E-301 is on leave today, so the network backup rotation took T-2001 and T-2013.
- E-202 is on leave on the 26th, so the business-apps backup rotation took T-2002.
Business hours:
- The sev3 and sev4 deadlines start counting when the desk opens at 09:00.
- The sev4 deadlines also skip the company holiday on Wednesday 25 March.
- sev2 deadlines run on the clock from 08:10.
Other notes
- T-2001 (Berlin) and T-2013 (Lisbon) were not merged. They are different offices with different symptoms. It may still be worth checking whether they share a cause.
- No ticket had company-wide impact, so no major incident was opened and no pages were sent.
- A comment explaining the decision was left on each ticket with unusual routing or severity.
This summary comes from one worker's report, and I could not check it independently against the system.
