Did the design run?
- Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
- No API failures: heldharness.apiNo model call failed at the API.
- Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
- No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
- Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
- No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
- Builtin tools: heldharness.builtin-toolsA check recorded with the run.
- Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
- No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
- The plan was a plan: heldplan-execute.planThe planner’s reply was read through its schema and held at least one step.
- Each call was shown the right tools: heldplan-execute.toolsThe planner was shown no tools and the executor the task’s full toolset.
Phases
Drawn from the phase records: timing and cost per call, no tool calls or messages.

The call-by-call replay is not published for this run. Traces are large, so the repository keeps 9 of this run set’s 900 and leaves the rest out; the score, phases and final answer on this page come from the committed run record.

Final answer
I've opened incident INC-1 for the company-wide CI outage and run it by the current major-incident runbook (KB-40). Every runbook step is done. Two notes first:
- Desk clock: it reads 2026-03-30 15:20 UTC, not the 2026-10-13 the plan assumed, so I used the desk clock.
- Runbook version: KB-39, the 2025 edition, is marked superseded by KB-40, so I ignored it. No knowledge-base article amends the severity matrix.
Runbook steps:
- Incident: INC-1 was opened at 15:20 UTC, before anyone was contacted.
- Commander: I paged E-401 Priya Raman, who is on call for incident-command now and not on leave, and made her commander. She is the only person paged, once.
Linked reports: I linked nine open reports of this outage to INC-1: T-3101, T-3102, T-3103, T-3104, T-3105, T-3106 and T-3109. T-3113 (filed under CRM) and T-3114 (filed under VPN) are also in, because both say CI has no runners available.
- Left out: T-3107 (a flaky payments test reported before the outage), T-3108 (one person's local Xcode build) and T-3110 (slow CRM dashboard). None of them was changed.
Severity: all nine linked reports are now sev1, and every deadline already set on them is unchanged.
- T-3106 and T-3109 had no deadline, so they got the triage time plus the 60-minute sev1 target: 16:20 UTC.
- Comms lead: the comms rota names E-801 Mia Laurent, but she is on sick leave until 31 March. The backup rotation (comms-secondary) covers, so I named E-803 Elif Demir, who is on call there now.
- Status update: posted to the status page as "Impact: All CI builds company-wide… Started: 14:52 UTC. Next update: 15:50 UTC." The next update is due at 15:50 UTC, which no one has been asked to cover yet.
- Postmortem: I created T-3115, linked to INC-1 and due 2026-04-08 at 17:00 UTC. That is close of business on the 5th business day; 3 April and 6 April are company holidays and don't count.
Where I didn't follow the plan:
- No merging: the runbook says to link every open report to the incident and governs here. Merging would have closed the reports it says to link, so they all stay open and linked.
- Postmortem owner: I left T-3115 unassigned. The owning rotation (platform-primary) has no published shift at its due time, and I didn't want to guess.
- No extra assigning or escalating: the runbook doesn't ask for it, and the commander already holds the incident. So the linked reports' owners are as they were; only T-3101 has one (E-101 Dana Whitfield).
Nothing was closed: there were no spam, auto-reply or security tickets in the queue, and the incident has open linked tickets.
