Did the design run?
- Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
- No API failures: heldharness.apiNo model call failed at the API.
- Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
- No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
- Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
- No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
- Builtin tools: heldharness.builtin-toolsA check recorded with the run.
- Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
- No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
- The plan was a plan: heldplan-execute.planThe planner’s reply was read through its schema and held at least one step.
- Each call was shown the right tools: heldplan-execute.toolsThe planner was shown no tools and the executor the task’s full toolset.

Final answer
I combined the three reports of the CRM search outage into one ticket, T-1052. It's now graded sev2 and assigned to Ines Kovac (E-201), with a deadline of 2026-03-17 12:40Z.
Merged into T-1052: T-1053 and T-1054. All three report the same thing: account search in the CRM returns an HTTP 500 server error on every query, starting around 07:00Z, all from Revenue Ops. T-1052 was the first report (created 07:05Z), so it's the one kept.
Final state of T-1052:
- Impact: department. T-1053 says the whole department is blocked, which is the widest impact reported.
- Severity: sev2, which the escalation matrix gives CRM for department-wide impact.
- Deadline: 12:40Z. This was already T-1053's deadline and was the earliest one promised on any of the three, so T-1052 took it over from its own 15:05Z. It is earlier than T-1052 had, but it comes from an existing promise rather than from re-grading.
- Assignee: Ines Kovac. Her team (business-apps) owns the CRM, and she is on call there at 12:40Z.
- Links: the engineering tracking ticket T-1061 used to link to T-1054 and now links to T-1052. No links to the merged tickets remain.
- I left a comment on T-1052 recording all of this.
Left alone:
- T-1055 (CSV export gives an empty file) is a different problem. It's a single user's report, and the requester says search works fine for them.
- T-1061 (search index rebuild) is an engineering tracking ticket created the day before the outage, not a report of the incident. Only its link changed, as part of the merge.
Decision for you: I didn't escalate the ticket, though the plan said to. The matrix only says where a sev2 goes if it is escalated: the platform-primary team, where Dana Whitfield (E-101) is on call at 12:40Z. The task asked for the ticket to be graded and assigned, and escalating would take it away from the CRM's owning team. If you want it escalated, it would go to Dana Whitfield and the deadline would stay at 12:40Z.
