Did the design run?

every check held
  • Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
  • No API failures: heldharness.apiNo model call failed at the API.
  • Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
  • No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
  • Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
  • No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
  • Builtin tools: heldharness.builtin-toolsA check recorded with the run.
  • Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
  • No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
  • Each call was shown the right tools: heldsingle.toolsThe agent was shown exactly the task’s tools, none missing and none extra.

The call-by-call replay is not published for this run. Traces are large, so the repository keeps 9 of this run set’s 900 and leaves the rest out; the score, phases and final answer on this page come from the committed run record.

Final answer

I opened INC-1 for the CI outage and ran the current major-incident runbook (KB-40) step by step. KB-39 is an older edition that KB-40 replaces, so I didn't use it.

  1. Incident opened: INC-1 for build-pipeline at 15:20 UTC, before contacting anyone.
  2. Commander paged and named: I paged Priya Raman (E-401), who is on call for incident-command right now, and made her commander. The handbook says to route by ticket deadline, but the runbook says to page whoever is on call now. The runbook governs major incidents, so I followed it. The page has already reached her and can't be taken back.
  3. Reports linked: T-3101, T-3102, T-3103, T-3104, T-3105, T-3106 and T-3109 are all reports of runners not picking up jobs. I left two tickets out on purpose because they are separate problems:

    • T-3107: a flaky payments-service test, which a knowledge base article (KB-41) says is unrelated to runners.
    • T-3108: a local Xcode build failure, which KB-42 says doesn't involve CI.
  4. Raised to sev1: all seven linked reports. Deadlines already promised to requesters stay as they were. T-3106 and T-3109 had no deadline, so each now has one at 16:20 UTC (triage time plus the 60-minute sev1 target).
  5. Comms lead: the comms rota names Mia Laurent (E-801), but she is on sick leave. Elif Demir (E-803), on call for the backup rotation (comms-secondary), is comms lead instead.
  6. First status update (on the company status page): "Impact: all CI builds company-wide are failing… Started: 14:52 UTC. Next update: 15:50 UTC."
  7. Postmortem ticket: T-3115, linked to INC-1, due 2026-04-08 17:00 UTC. That is the 5th business day after the incident opened, skipping Good Friday (Apr 3) and Easter Monday (Apr 6).

Still open:

  • Next status update is due by 15:50 UTC. The runbook requires one within 30 minutes, and someone needs to post it.
  • No severity on T-3115: it was created as company-wide, which the matrix grades sev1. I didn't apply that because it's a postmortem rather than an outage report.
  • No reports merged or reassigned: the runbook calls for linking, not merging.
