Did the design run?

every check held
  • Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
  • No API failures: heldharness.apiNo model call failed at the API.
  • Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
  • No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
  • Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
  • No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
  • Builtin tools: heldharness.builtin-toolsA check recorded with the run.
  • Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
  • No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
  • Each call was shown the right tools: heldsingle.toolsThe agent was shown exactly the task’s tools, none missing and none extra.

The call-by-call replay is not published for this run. Traces are large, so the repository keeps 9 of this run set’s 900 and leaves the rest out; the score, phases and final answer on this page come from the committed run record.

Final answer

I've opened major incident INC-1 for the CI outage and run every step of the current runbook, KB-40. The 2025 runbook (KB-39) has been replaced by KB-40, so I didn't use it. INC-1 now has a commander, a comms lead, seven linked reports, a first status update and a postmortem ticket. The next status update is due at 15:50 UTC.

  1. Incident opened: INC-1 was opened at 15:20 UTC, before contacting anyone.
  2. Commander: I paged Priya Raman (E-401), who is on call for incident-command now, and made her incident commander. Per KB-40, this page went to whoever is on call now, not whoever is on call when a ticket falls due.
  3. Linked reports: I linked seven reports of the outage: T-3101, T-3102, T-3103, T-3104, T-3105, T-3106 and T-3109. T-3105 is filed as a single-user problem, but it says the job "never gets a runner", so it's the same outage. I left two tickets out because they are different problems:

    • T-3107 is a flaky payments test, reported before the outage began.
    • T-3108 is a local build that fails after an Xcode upgrade.
  4. Severity and deadlines:

    • All seven linked reports are now sev1.
    • Existing deadlines are unchanged, since a deadline given to a requester is never moved earlier.
    • T-3106 and T-3109 had no deadline. Each now has one: triage time plus the 60-minute sev1 target, so 16:20 UTC.
  5. Comms lead: The comms rota names Mia Laurent (E-801), but she is on sick leave today. Elif Demir (E-803), on call for the backup rotation (comms-secondary), is the comms lead instead.
  6. Status update: Posted to the company status page: "Impact: all CI builds company-wide are failing… Started: 14:52 UTC. Next update: 15:50 UTC." 14:52 is when the first report (T-3101) came in.
  7. Postmortem: Ticket T-3115 is linked to INC-1 and due 2026-04-08 at 17:00 UTC. That is close of business on the 5th business day, skipping Good Friday (3 Apr) and Easter Monday (6 Apr). I didn't set a severity on it.

I deliberately didn't do three things:

  • I didn't merge the duplicate reports, because KB-40 says to link every report to the incident, and it overrides the handbook during the incident.
  • I didn't reassign any tickets, because the runbook doesn't call for it.
  • I didn't touch T-3107 or T-3108.

The current desk time is 2026-03-30 15:20 UTC.
