Did the design run?
- Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
- No API failures: heldharness.apiNo model call failed at the API.
- Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
- No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
- Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
- No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
- Builtin tools: heldharness.builtin-toolsA check recorded with the run.
- Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
- No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
- The plan was a plan: heldplan-execute.planThe planner’s reply was read through its schema and held at least one step.
- Each call was shown the right tools: heldplan-execute.toolsThe planner was shown no tools and the executor the task’s full toolset.
Phases
Drawn from the phase records: timing and cost per call, no tool calls or messages.

The call-by-call replay is not published for this run. Traces are large, so the repository keeps 9 of this run set’s 900 and leaves the rest out; the score, phases and final answer on this page come from the committed run record.

Final answer
I ran the outage under the KB-40 major-incident runbook (KB-39 is marked superseded). I did not merge any tickets.
- Incident: I opened INC-1 for build-pipeline at 15:20Z, before contacting anyone.
- Commander: I paged E-401 Priya Raman (PG-1) and named her commander. She is the incident-command engineer on call now, and nobody else was paged. KB-40 says to route this page by the current on-call, not by a ticket deadline.
- Comms lead: E-801 Mia Laurent is on comms but on sick leave today. The backup rotation is comms-secondary, and E-803 Elif Demir is on call there now, so she is comms lead.
- Linked reports: T-3101, 3102, 3103, 3104, 3105, 3106 and 3109 are build-pipeline reports of the outage. I also linked T-3113 (CRM release) and T-3114 (VPN release). Both say CI has no runners, so I treated them as reports of this outage. If you would rather keep other-service tickets out, they can be unlinked.
- Severity: All nine linked reports are now sev1. Their committed deadlines are unchanged.
- Deadlines: T-3106 and T-3109 had none, so each got 16:20Z (15:20 plus the 60-minute sev1 target).
- Status update: The first update is posted: "Impact: all teams company-wide… Started: 14:52 UTC. Next update: 15:50 UTC."
- Postmortem: T-3115 is linked to INC-1 and due 2026-04-08 17:00Z. That is the fifth business day after the incident, counting Mon 3 Apr (Good Friday) and Mon 6 Apr (Easter Monday) as holidays.
Left alone:
- T-3107 is a flaky test and T-3108 is a local Xcode link failure. Neither is part of the outage.
- T-3110 is a CRM dashboard issue and T-3090 was already closed.
- I closed nothing, since nothing in the queue was spam or an automatic reply.
Still to do:
- The next status update is due by 15:50Z.
- Only T-3101 has an assignee (E-101). The other linked tickets are unassigned, and I did not assign or escalate them because the runbook doesn't call for it.
The task said today is 2026-10-06, but the desk clock reads 2026-03-30. I used the desk clock for all times and dates.

Cost excludes judge calls, which are tracked separately so a scoring model never flatters or penalises an architecture. Every figure on this site is read from results/ at build time.
