Did the design run?

every check held
  • Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
  • No API failures: heldharness.apiNo model call failed at the API.
  • Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
  • No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
  • Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
  • No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
  • Builtin tools: heldharness.builtin-toolsA check recorded with the run.
  • Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
  • No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
  • The plan was a plan: heldplan-execute.planThe planner’s reply was read through its schema and held at least one step.
  • Each call was shown the right tools: heldplan-execute.toolsThe planner was shown no tools and the executor the task’s full toolset.

Final answer

I opened and ran INC-1 for the company-wide CI outage, following KB-40 (the current runbook). KB-39 is marked superseded, so I ignored it.

  • Incident and paging: I opened INC-1 on build-pipeline before contacting anyone. I paged only E-401 Priya Raman, who is on call for incident-command now, and made her commander.
  • Comms lead: The comms rota names E-801 Mia Laurent, but she is on sick leave today. Under the leave rule I used the backup rotation, comms-secondary, so E-803 Elif Demir is comms lead.
  • Linked reports: I linked nine reports of this outage to INC-1: T-3101, 3102, 3103, 3104, 3105, 3106, 3109, 3113 and 3114.

    • T-3113 (CRM release) and T-3114 (VPN release) came in on other services, but both describe the same "no build runners available" problem, so I included them.
    • I left out T-3107 (a flaky test) and T-3108 (a local Xcode link failure), which are different problems.
  • Severity: I raised all nine linked reports to sev1 as the runbook says. Existing deadlines are unchanged. T-3106 and T-3109 had no deadline, so I gave them 16:20 UTC (15:20 triage time plus the 60-minute sev1 target).
  • Status update: I posted the first update: "Impact: All engineering teams company-wide cannot run CI builds or releases (no build runners available). Started: 14:52 UTC. Next update: 15:50 UTC." The 14:52 start is when the earliest report, T-3101, was created.
  • Postmortem: I created T-3115, linked to INC-1 and due 2026-04-08 17:00 UTC. That is the close of business on the 5th business day after today, counting Mar 31, Apr 1, Apr 2, Apr 7 and Apr 8, with Apr 3 and Apr 6 skipped as company holidays.

I did not merge the duplicate reports. The runbook only asks for linking, and merging would have folded the CRM and VPN tickets into a build-pipeline ticket. The handbook also says to change only what the request is about. I didn't change any assignments or close anything.

Still pending:

  • Status updates: The next update is due by 15:50 UTC, and E-803 needs to post it.
  • Ownership: T-3101 stays with E-101 (platform-primary). The other reports have no assignee.
  • Merging: Say so if you want the duplicates merged, with T-3101 as the survivor.
