Did the design run?
- Run completed: heldharness.run-completedThe environment was built, the architecture returned and grading finished, none of them throwing.
- No API failures: heldharness.apiNo model call failed at the API.
- Every call ended: heldharness.calls-endedEvery call ended with a result, or was stopped by the run's own limits.
- No harness tool denied: heldharness.harness-toolsThe sandbox guard never denied a harness tool, nor a tool the session itself offered.
- Tool servers connected: heldharness.mcp-connectedEvery tool server the harness gave a call was connected when the session started.
- No foreign tool servers: heldharness.mcp-hermeticNo call loaded a tool server the harness did not give it.
- Builtin tools: heldharness.builtin-toolsA check recorded with the run.
- Structured replies arrived: heldharness.structured-outputEvery call that asked for structured output and ran to the end got it.
- No silent fallbacks: heldharness.not-degradedNo phase fell back on a default (an empty plan, a default winner) instead of its output.
- All three proposers ran: heldpropose-select.proposalsThree proposers ran in parallel, each from its own angle, and none was starved while another ran.
- The selector chose: heldpropose-select.selectionThe selector picked a proposal by a valid number, read from its schema reply and not a default.
- Each call was shown the right tools: heldpropose-select.toolsEach proposer was shown the read-only tools, the selector none, and the executor the full toolset.

Final answer
I ran the KB-40 major-incident runbook for the build-pipeline outage as INC-1. The state matched the plan, so I made no changes to it.
- Runbook: KB-40 is current and KB-39 is superseded. I used none of the KB-39 steps.
- Incident: I opened INC-1 before contacting anyone.
- Commander: I paged E-401 Priya Raman, who is on call for incident-command now, and only her. She is now incident commander.
- Linked reports: T-3101, T-3102, T-3103, T-3104, T-3105, T-3106 and T-3109 are linked to INC-1. I did not merge them.
- Left alone: T-3107 (a flaky test that passes on retry), T-3108 (a local Xcode link failure) and the closed T-3090.
- Severity: all seven linked reports are now sev1, per the matrix (build-pipeline, company-wide). T-3105 is a single-user report, but its job never gets a runner, so I included it.
- Deadlines: the five existing deadlines are unchanged. T-3106 and T-3109 had none, so each got 16:20Z, which is triage at 15:20Z plus the 60-minute sev1 target.
- Comms lead: E-801 Mia Laurent is on sick leave on 30–31 March, so cover goes to the backup rotation, comms-secondary. I named E-803 Elif Demir, who is on call there now. This was a role assignment, not a page.
- Status update: posted at 15:20Z: "Impact: all engineering teams company-wide; CI builds failing, no healthy runners. Started: 14:52 UTC. Next update: 15:50 UTC." The start time is the earliest report, T-3101, and the next update is due within 30 minutes of posting.
- Postmortem: T-3115 is linked to INC-1 and due 2026-04-08 17:00Z, the close of the fifth business day after 30 March. Good Friday (3 April) and Easter Monday (6 April) are company holidays and don't count.
Two things for you to do:
- Next update: it is due at 15:50Z.
- Postmortem severity: T-3115 shows severity "unset". The runbook doesn't ask for a severity on it, so I left it.
